A 61-year-old man with known stable angina presents with a 2-week history of chest pain that has changed pattern — now occurring at rest for up to 30 minutes, radiating to the left arm, jaw and neck, associated with nausea and dyspnoea at rest. Working diagnosis is unstable angina (NSTE-ACS); NSTEMI, stable angina, GORD, musculoskeletal chest pain and anxiety must be considered. The station rewards a structured ACS history, TIMI risk scoring and prompt antiplatelet and anticoagulation management.
