A 16-year-old known type 1 diabetic presents to the emergency department drowsy and confused (GCS 9) after abdominal pain and vomiting, having likely missed insulin. Working diagnosis is severe diabetic ketoacidosis (pH 7.00, ketones +++, glucose markedly raised); hypoglycaemia, shock and other causes of reduced consciousness must be excluded. The station rewards a structured DRSABCDE primary survey and prompt DKA management.
