A 52-year-old man presents with 6 weeks of bilateral foot pain (worse on the balls and toes, worse in the mornings), left knee swelling, and what he describes as chilblain-like changes on his toes, onset since starting a new job with safety boots on cold floors. Working diagnosis is psoriatic or reactive arthritis; gout, RA, OA and septic arthritis must be actively excluded.
