A 74-year-old woman brought in by her daughter with a 6-month history of low mood, anhedonia, biological symptoms of depression and passive suicidal ideation following bereavement, with mood-congruent somatic (cancer) and poverty delusions. Working diagnosis is a major depressive episode with psychotic features in an older adult; complex bereavement, organic causes (dementia, delirium, Parkinson's, stroke) and bipolar disorder must be considered.
